Counselling Contract

Before the beginning of our work together it is important for us to have a working agreement, so that we both know what is required of us.  Please carefully read the following information then email me at counsellinginyourcommunity@gmail.com to say that you understand and agree before our first session.

My name is Hazel Hill and I am a qualified registered Senior Accredited (45039) Counsellor and a Member of the British Association for Counselling and Psychotherapy (BACP). I work to BACP’s ‘Ethical Framework for Counsellors’ and hold professional indemnity insurance and a current DBS certificate. 

Conditionality

My counselling work is confidential. The only time that I may break confidentiality is:

  • if you are at serious risk of causing harm to yourself or others
  • child abuse is disclosed
  • you share information about proposed act of terrorism
  • when I am compelled to do so by law

If any of the above situations occurred, I will discuss with you beforehand on how I will disclose this information. If at any point during our counselling arrangement I felt that you needed emergency support, I may ask for your consent to contact your GP. 

Supervision

I have regular clinical supervision with a qualified professional where I discuss my work with clients in confidence. This work is anonymous.

Session

We will agree a time and date for our session after each individual session. Sessions will last for 50 minutes.  They will be held at my office at 59 Wostenholme Road. Our sessions will be open ended with regular 6 weekly reviews and us agreeing the date for our next appointment at the end of each session. You may stop our sessions at any time.

Notes

I may take brief notes after each session. These will be kept in accordance with BACP Ethical Framework and General Date Protection Regulation (GDPR) 2018. You are welcome to request to see the notes at any time. These notes are securely stored, and you cannot be identified by them. Please read my privacy policy.

Fees and Payments

Individual video sessions are £65 and couple counselling is £80. £5 extra per session for evening appointments. Each session needs to be paid 5 days prior each appointment.

Payment is via direct bank transfer. (Bank Account in my name, 30-93-17, A/c No: 02476828). Please use your first name as a reference and let me know by email or text that you have made the payment. All subsequent sessions need to be paid 5 days in advance.

Cancellation Policy 

Please give as much notice as possible for cancellations/postponement (text 07814 363855 or email counsellinginyourcommunity@gmail.com). If you cancel with less than 5 days notice, you will be charged for that session.

Contact outside of session

I will only accept contact outside of the session for practical administration reasons. I will not enter into email or telephone counselling without any prior agreement in our session. 

I do use social media but will not accept social networking requests or chat with you through this medium as I believe it will comprise our counselling relationship. Please look at my social media policy for more information. If we meet outside of the counselling session, I will not acknowledge you. You are welcome to say hello to me but I will not discuss the content of our sessions. 

Note taking

I will keep record of the time and date you attend. These will be held anonymously for 7 years (in accordance with my insurance company. Twelve months after our sessions terminate, I will delete any emails, texts and your contact details.

Complaint

It is my most sincere hope that you would not have cause to complain about my practice. If you are dissatisfied with my services, please raise this matter with me in person or in writing. I will acknowledge the complaint and if necessary, consult with my supervisor. I will respond to your complaint within 7 working days as well as invite you to discuss it with me in person
